At Medlife Diagnostic Laboratories, Inc., (hereafter “MedlifeDiagnostic” or simply “Medlife”), accessible from https://medlifediagnostic.com (the “website”), one of our main priorities is the privacy of our users. This Privacy Policy document contains types of information that is collected and recorded by Medlife’s Software Applications and how we use it in order to offer you a professional experience.

Patient User → Any person who visits the Medlife website either for informational, commercial or professional purposes.
Phlebotomist User –> Any person who uses the Medlife system in order to execute mobile phlebotomy appointments.
Services → The professional services that are offered by “Medlife” to its users
Content → Any form of intellectual property that is published by medlife on the website, in any digital form (illustrations, images, articles, videos, sound recordings, texts, source code, animations)
Data → Any form of information or content that is provided by the user or generated by medlife in order to ensure the set of features exposed by the medlife website and applications to its users.
Consent
By using our software solutions (web or mobile applications), you hereby consent to our Privacy Policy and agree to its terms.
Distinct Types of Users
The software Application operated by Medlife facilitates the interaction between two distinct types of users:
- The Patient User, or the person that download and uses the Medlife application in order to schedule mobile phlebotomy appointments.
- The Phlebotomist User, or the hired Medlife professional that uses the Medlife application in order to execute mobile phlebotomy appointments
Information We Collect on the Patient User
The personal information that you are asked to provide, and the reasons why you are asked to provide it, will be made clear to you at the point we ask you to provide your personal information.
If you choose to use our mobile application or web applications, we may require you to provide us with certain personally identifiable information as part of your booked appointments. The collected information can be found as follows:
- Your personal information (first name, last name, date of birth)
- An avatar photo
- A photo of your personal ID
- A photo of your active medical insurance
- A photo of your recent prescription for performing blood-work
- An address or location from which you would want the mobile phlebotomy appointment to be performed
How we Use the Information collected from the Patient
This information is used solely for regulatory compliance purposes and for facilitating the mobile phlebotomy services put forth by Medlife. The documents and information that you submit as part of an appointment or your profile are used for:
- Identifying you personally
- Verifying your active medical insurance
- Assessing your medical prescription
- Scheduling your appointment with us.
- Processing your blood draw requests
- Offering you a more personalized experience
The data that you provide is used in order to process your blood-work while offering best in class quality of service in offering blood-work diagnostics services.
Information We Collect on the Phlebotomist User
The phlebotomist user is a professional user that works in the service of Medlife. The information that is collected about this type of user is:
- The personal information (first name, last name, date of birth)
- An avatar photo
- The precise location while executing a mobile phlebotomy appointment
- The route history and appointment history for the performed mobile phlebotomy appointments
How we Use the Information collected from the Phlebotomist User
This information is used solely for regulatory compliance purposes and for facilitating the mobile phlebotomy services put forth by Medlife. The information that is collected about the phlebotomist is used for:
- Operating the map navigation feature of the Medlife mobile phlebotomy app
- Showing your coordinated to the Patient User in realtime
- Verifying routes performed by the phlebotomist on appointments that have been booked through the app
- Reporting statistical information about these performed routes in order to improve the overall quality of service
General use of information
The general purposes for collecting data through the Medlife platform is to:
- Provide, operate, and maintain our mobile application
- Improve, personalize, and expand our mobile application
- Understand and analyze how you use our mobile application
- Develop new products, services, features, and functionality
- Communicate with you, either directly or through one of our partners, including for customer service, to provide you with updates and other information relating to the mobile application, and for marketing and promotional purposes
- Send you emails
- Find and prevent fraud
- Log Files
Medlife’s Software Applications follows a standard procedure of using log files. These files log users when they use our applications in order to track and report suspicious behavior through the system. The information collected by log files include:
- Unique user identifier
- Internet protocol (IP) addresses,
- Device type
- Internet Service Provider (ISP)
- Date and time stamp
- Referring/exit pages
- Possibly the number of clicks.
These collected pieces of information are not linked to any information that is personally identifiable.
Cookies and Web Beacons
Like any other website, Medlife’s Software Solutions uses ‘cookies’. These cookies are used to store information including users’ preferences, and the pages on the application that the user accessed or visited. The information is used to optimize the users’ experience by customizing our web page content based on users’ browser type and/or other information.

This section establishes the circumstances in which we will reveal your data to third parties and any other supplementary purposes for which we use your data. We use a series of service providers that are important for the development of our business and that process your information for us and on our behalf, respectively:

Indicating possible criminal acts or threats against public safety to a competent authority.
If we suspect that criminal behaviour or potential criminal behaviour took place, we will need, in certain circumstances, to contact a competent authority, such as the police. This could be the case, for example, if we suspect a fraud or a cybernetic crime has been committed or if we receive threats or malicious communications towards us or towards third parties.
In general, we will have only to process your information for this purpose, if you were involved or affected by such an incident in some way.

User Rights
CCPA Privacy Rights (Do Not Sell My Personal Information)
